Describe the routine

List the practical details a willing helper may need: familiar routines, school or care contacts, and where important supplies are kept. Keep the information current and limit sensitive details to what is necessary.

Ask about willingness

Discuss possible support with the people you have in mind. Consider their circumstances and capacity. A person named in a planning note has not automatically agreed to help or acquired decision-making authority.

Prepare questions about formal care

If you want to record guardianship or another formal arrangement, seek qualified local advice. Use your LastConnect worksheet to organise questions and preferences, not as evidence that an appointment is legally effective.

Revisit the plan

Update the record as children grow or an animal’s needs change. Check the contact details and storage locations for relevant records. A useful plan should be easy to understand and proportionate to the task.
